Theodore A. Boutacoff

THEODORE A. BOUTACOFF, co-founded IRIDEX and since February 1989 has
served as its President, Chief Executive Officer and a member of its
Board of Directors. Prior to co-founding the company, Mr. Boutacoff held
various positions, including Director of New Business and Clinical
Development, Director of Marketing and Director of Regulatory Affairs,
with the Medical Division of Coherent, Inc., a manufacturer of laser
systems for science, medicine and industry. Mr. Boutacoff holds a BS
degree in Civil Engineering from Stanford University.
